feat: Review Session增加Fix进度跟踪卡片,移除独立Dashboard模板

- 新增Fix Progress跟踪卡片(走马灯样式)显示修复进度
- 添加/api/file端点支持读取fix-plan.json
- 移除review-fix/module-cycle/session-cycle中的独立dashboard生成
- 删除废弃的workflow-dashboard.html和review-cycle-dashboard.html模板
- 统一使用ccw view命令查看进度
